Back to School Immunization Clinic
The Northeast Texas Public Health District will be administering back to school immunizations at the middle schools shown below. By taking advantage of these clinics, you will not have to be the ones to sit for extended periods of time, waiting for a vaccination that you could have already obtained. “DON’T WAIT, VACCINATE’

Those kids who are on Medicaid and CHIP will need to present their cards at the time of service and there is no charge for the vaccines. Those who do not have any form of insurance will be assessed a $10.00 administration fee for each vaccine at the time of service. This payment can be in the form of a check or cash.

|IMPORTANT INFORMATION FOR ALL 6th GRADE STUDENTS AND PARENTS:|
|NOW is the time for 6th grade parents to bring an updated copy of your student’s immunization record to the SCHOOL NURSE---IF YOU HAVE NOT DONE THIS YET.
___Tdap (tetanus-containing vaccine)- within last 5 years
___Varicella #2-Exception: if student had previous illness- must provide written statement from physician or parent
Your student NEEDS to have record of the ABOVE listed immunizations for school attendance. Please bring the updated immunization record from your Doctor or Clinic to the school or have your Doctor’s office fax them to me. I will call you when I get the fax. Please call me or email me if you have any questions. Many times the student has had these immunizations, but the school does not receive the updated record from you—so please check. I will be happy to check the immunization record that I have on file for your student. Thank you for your co-operation.
